Create new form with auto filled fields

I have a dynamic pdf form that is used for applications. Would it be possible to have a button that when clicked opens or generates another form with certain fields already filled out (name, address, etc.) from the application form?

 

I could maybe add the evaluation to the first form and have it hidden until the button is clicked? Is that possible? The application form has a lot of addInstances already, so I don't know if that matters.

 

Any advice you can give will be greatly appreciated.

@fogharty-1  I will try to create a sample that could serve the purpose- will share with you.

    @fogharty-1 Not a great idea to pop another pdf from inside a pdf that too with data(browsers do not support xfa based form), this will fail a lot. If this was being rendered as HTML, then certainly possible.
