Can not create Adaptive Forms by custom template

Hi All,

I want to create the form on local SDK and I followed this site to create an adaptive form.

I have created an Adaptive Form Template, but I cannot create form by this template.

but I found that Blank and Basic Template is ok.

Do you have relevant selection made in the Style/Data/Submission tab while creating the form?

The create button will be greyed out if the options in any of these tabs is unselected OR there is a permission issue with the user.
