AEM 6.5.20 with Forms-addon Package (AEM OSGI form) not wo

Hi Team

 

We are working AEM OSGI forms Project with AEM6.5.17 . Recently we updated to AEM6.5.20 corresponding AEM OSGI Forms ,But when we creating Adaptive form using Blank template Form is not created getting Console errors . Please let me know if any workaround & known issues for AEM  OSGI Form Package for AEM6.5.20

 

Installed following package to upgrade from AEM6.5.17 to 6.5.20

aem-service-pkg-6.5.20.0.zip

ADOBE-AEMFD-WIN-PKG-6.0.1192.zip

 

Here is Console error when click on "Create" Button :

Uncaught TypeError: Cannot read properties of undefined (reading 'isEnabled')
at HTMLButtonElement.<anonymous> (createguidewizard.js:242:41)
at HTMLButtonElement.dispatch (jquery.js:5232:27)
at elemData.handle (jquery.js:4884:28)
