Cannot Create a Review task on a DITA topic in AEM

Question

When I try to create a review task, no task is created. I'll walk through my steps:

1. Select "create review task:

2. Fill out the "create review task" form and assign the task to an existing project "review training repository topic."

3. Click "create." I get a dialogue that says "success! Review task 'test review' has been created. "

4. When I go to the project, no review has been created. No one on the project receives a notification either.

Any idea why a review task is not created?

@davidl38471046 Please check if one of these conditions are being reached, which would close the review tasks if the individual topic is closed, all its review tasks are closed.
