I recently implemented IMS authentication for the Real-Time Transaction Server API.
It was quite challenging for us too—it didn’t work the first time either. I’m not sure where your Adobe Campaign (AC) instance is hosted—on Adobe Managed Cloud or in a hybrid setup—but I can share what we discovered during our troubleshooting process.
After working with Adobe Support, they helped us identify a few key issues:

Make sure your IMS connection is correctly configured in the External Account, with all parameters properly filled.

Ensure that each user has the necessary permissions.

If you’ve set up a Developer Project in Adobe Developer Console, verify that it’s associated with the correct Product Profile.
I would recommend reaching out to Adobe Support. As Campaign developers, we can only configure what’s within the Adobe Campaign UI. IMS-related configurations often require internal Adobe support, as they are set up at the organization level. It’s unlikely we, as Campaign developers, have access to those settings.
