Project custom form, new information should be dated and highlighted

When a request gets converted to the project and the custom form data is transferred over to the project, we don't allow anybody edit the project custom form as it is difficult to know what was changed from the original request. We'd like the requester or anybody in the project to be able to add to the custom form and have this highlighted with the name of who made the change and the date that was changed.
