Custom Forms Attaching Automatically When Converting Issue/Request to a Project where no project template is being used

When filling out a custom form via a request queue and converting the issue/request to a project (where there is no project template) we would like the custom form to attach automatically without a team member having to manually attach the custom form to the project for the information to flow over.
