I Have a few questions on customer attributes:

Does the name of the loaded file always have to be the same?
Scenario 1 - What happens if I add an additional column to our Customer Attribute file?
Scenario 2 - What happens if I create a new Customer Attribute, but I assign it the alias ID that I used to match with ECID?

It can be same or different. Target always merge with the primary key that you selected.

If it find a match of an existing entry it merge the record else it will add as new record
