"Invoke an AEM Workflow"

I'm attempting to invoke an AEM workflow on form submission.  I have set this option on my adaptive form and specified the workflow model, but when I submit the workflow is not invoked.  Am I missing something?  Just trying to setup a basic proof of concept where a user submits a form and it routes to supervisor for approval.  Any assistance is appreciated.

If you're following this and its still not working, what are the logs in the error.log that you're getting. That might reveal something useful.
