Attach custom form through fusion

I am trying to attach custom form to a document. Please help!

Just to complement:

Hello Elan,

I would try to do it the other way round:

- add the module Misc Action

- Record Type => Document

- Action => assignCategories
- objID => <documentID>

- objCode => DOCU

- categoryIDs => <ID of your custom form>
