Value of task custom form field is blank

I have a custom form attached to a task.  I’m using the same custom field used on the Request form.  The form object type is (Project, Task, Request).  I want to show the value added to the request form into the task form (Task Details).  It works when I add the form to Project Details. However, I get blank fields on the Task Details custom form.
