Text Mode: How to display Document Approval Status and Decision in a report?

I need to add columns to a report to display real-time document approval Status and Decision values (such as "Approved", "Needs work", or "In review").

Currently, these columns are showing up completely blank for several document rows, even though I can see active workflows and user decisions when viewing the files directly in the Workfront interface.

I have already tried building Document, Document Version, and Document Approval reports, but I cannot locate the correct database fields. Standard Text Mode fields like approvalStatus and proofDecision are remaining empty.

Could someone please provide the exact Text Mode code or the specific API fields/bridge objects required to map these active workflow decisions into a report?
