Print a blank Custom Form

Does anyone have a good way to print a blank Custom Form?  

We have a business case Custom Form.  We're making a physical project management board of excellence, and I need to print a physical copy of this Custom Form to put there.  

I opened the Custom Form in the editing tool and went to Preview.  Normally I use PicPick for a scrolling screenshot for this type of thing, but the giant "End Preview" bar at the bottom of the screen is throwing off the software, and the scrolling screenshot doesn't look right.  I tried in Chrome, Firefox, and Edge.  

Does anyone have any other ideas?  I could make a dummy project to attach the Custom Form to and leave the whole thing blank, but that has the "+Add" buttons at each field and doesn't look as good.
