Is there a way to report on the number of times a custom form has been used?

Looking to find out how many times a specific custom form has been used. Is this possible?

    If you're looking for 1 specific custom form... I would create either a project or issue report and filter by "Categories >> ID" - this will give you any project/issue where that form is used.

    Note that "Category >> ID" will on pull those projects/issues where your specific form is the first form on the project/issue
