Is there a way to notify a specific user when a custom field (check box) is ticked off within an Issue Custom form?

We have an Issue Custom Form and a field in there named IA Extension - check box. Is there a way to notify a specific user when this field is ticked off within an Issue Custom form?

yes, you can definitely do that with Fusion. 
If you want to rely on Workfront alone, the best I can think of is to setup report that would list required issues and setup scheduled delivery of that report to specified user on regular cadence.
