Kevin: no answers, but I think it's worth noting we've discovered similar problems when searching for custom forms in (or rather, not in) templates. Here's a summary of everything we tried. Interesting. As my troubleshooting capacity is limited and responses here are delayed, I would ask you to open a support request so we can dig in deeper into specifics for your instance. I would guess it's something as simple as syntax, but without access to your environment, it's hard to say.
