How do I get rid of that breadcrumb that was added to the request queues that shows "Recent Paths"? As I make updates to custom forms, it directs the project owners to the previous form version requiring that upon conversion, we are missing fields.

We then have to fill this in as planners to finish conversion. Also - newer users keep going back to whatever form they used previously instead of using the dropdown leading to the wrong request form.

I'm not sure what you're doing to have this happen. There is no such thing as a previous form version (unless you are specifically copying a form and making it into a new version): any changes I make to a form are immediately active.

Moreover, if I delete the queue topic or unpublish the queue, both actions cause the line to disappear from Recent Paths.
