How do I create a report that pulls comments?

I'm looking for some reporting on proofs that doesn't seem to be out of the box. I would like to create a couple of reports.

Report 1

I would like to pull a report that is just a dump of proofs with comments. This will allow me to do some word analysis to look for common proofing themes or issues. It would be ideal if they could be grouped by creator.

Report 2

I would like to run a proof report grouped by creator and grouped by decision that will allow me to make a stacked bar chart that shows the number of proofs by user and the number of of each decisions.

It looks like there are some options with custom field values but I'm not completely familiar with those custom query fields.
