Embedded request form on dashboard - UI improvements required

When we embed request form on a dashboard - which in itself is a great feature - some of the dropdown menus don't work well, e.g. mutliple scrollbars show up, users are not able to easily select items from the dropdown.

It seems like this could have been programmed a bit better to avoid those issues.
