Does anyone have score card examples that they are using to score project requests and are willing to share?

We are starting our Demand Management journey and are considering the use of score cards to help us "make sure the cream rises to the top." Anything you are willing to share that we can learn from will be much appreciated.

    Hi - getting the actual scorecard to show in a single screenshot was hard, but here is an example in a spreadsheet view
