Custom Form - PDF field - Access Denied

How do I get the PDF form to show on the custom form without getting an access denied error message? 

 

I have added the PDF field to a custom form - when I view the custom form, I receive an error message Access Denied. 

The PDF is in the request queue project documents. 

 

The custom form includes the PDF field - the URL is from the PDF location in the request queue project. 

I added a URL field to the custom form, when clicked to view PDF, Access Denied error message displays.

I don't see a "URL field" to put on my custom forms, but I put the PDF widget on there and was successfully able to get the PDF to show up. It wasn't particularly pretty (it was too small to read), and I don't think I'll be able to do anything with it, but it definitely didn't throw an error.
