Attaching reference docs (PDF, Excel, etc.) to Custom Forms

Allow the ability to view a reference document (like an Excel file or a Word document) that will be needed to complete a custom form.

The ability to add a hyperlink is useful, but having to house the file elsewhere makes it a challenge if there are multiple documents.
