Marketo Sales Insight (MSI) and MS Dynamics CRM: Unable to 'Send Marketo Email'

We've been trying for over a year now to get MSI deployed properly in our Dynamics CRM environment.  Our latest issue involves the 'Send Marketo Email' feature.  We can see the list of emails that we've pushed into MSI, but we cannot send anything.  When looking at the source code of the email (from within the MSI interface), much of the HTML code has been stripped or re-written, which is probably why nothing can be sent.  The email doesn't even render properly in the preview window of MSI.  This case has been with Support for a couple weeks now and it's still being worked on.

Are there any Dynamics CRM/MSI users out there that have gotten this feature to work properly within their environment?

I'd also be interested in getting input from SFDC users.  Do all of your emails render (in the MSI window) exactly as they do in Marketo?  Notice how all of the styles are completely stripped and we're left with a completely unformatted email:
