Form Embed Code with Hyperlink

I wanna make it so when someone clicks on a link in a landing page, a form pops up in a lightbox.  Does anyone have ideas on HTML coding to accomplish this?

If you search for "lightbox" I've posted several permutations of this (button to lightbox, lightbox of preloaded form for performance, etc.).
