is the unsubscribe form included in the "filled out form" filter for reporting?

Yes. Any form that is available can be used/seen in that filter.

It is common to set triggers to activate on any form EXCEPT for unsubscribe.

    They're there, can be tough to find them sometimes.  My original system form is named 'Email Unsubscribe Form'.

    Josh is correct.  If you use the filled out form filter set to "any form" it will include leads who filled out the Email Unsubscribe form.  You can pick and choose which forms you want to report on in that filter and exclude the unsubscribe form.

    Sorry - I was confusing with the behavior of the Clicks Link in Email filter.
