Conref note appears in AEM Guides web editor author mode but in Preview mode or published PDF

We have a conref note in a topic that displays in the AEM Guides web editor author mode but does not display in Preview mode or in our published PDF. While we have two outputclasses set on different element tags in this topic, neither outputclass excludes anything from the topic. We can't figure out why this behavior is happening.
