How to use Radio buttons with Image in Adaptive form?

Hi, I've requirement to use image as radio button in my AF.
So, as the titles says, is it possible to do that in adaptive form? I want to achieve something like below with small piece of text below image:

Can someone please give suggestion to accomplish this in AF, if possible?
If you have some other approach to achieve this, please share.

we have image choice as a component in AEM Forms. Have you tried using that?

Image ChoiceLets your customers select an image to provide information. You can use the information to provide personalized services to your customers.

    Oh yes. That component seems suitable, just need to add some styling to it. But I couldn't find any field to add text below any image option. Any input on that?
