Associate form template as a DoR Template NOT showing my DoR template

I have build an adaptive form, I have the DoR for the form, now I want to associate form template as a DoR template but when clicking on the raido button "associate forms template with the DoR template", my DoR that's on the crx does not appear, instead it brings other DoR template such as Geometrixx finance DoRs.

How do I make it appear as one of the options to select from?

Can you please explain what you mean by this? Do you mean your template?

Have you uploaded your template for Dor from Forms and Documents console in AEM Forms?

I uploaded the template in the UI, Create>> files from local storage
