This issue is resolved

Issue Type : Forms Addon Installation Issues.

Resolution : I have restarted AEM, and then reinstalled the forms add-on package on both the author and publisher which resolved it.

@gourav_mukherjee, can you please check if all bundles on your server are active? Secondly, what is the AEM Forms version are you using?

@gourav_mukherjee  Although you have checked the bundle's page, could you check the status/version of the CCM CCR Core Bundle? Is it consistent with the other CCM bundles? Just want to ensure that all dependencies for the service are correctly installed and configured.

Have you made some recent custom changes to the CM module? The service alias might have been changed or updated, and the server references an outdated one.

Can you share the complete error logs post a server restart?

@gourav_mukherjee Thanks for confirming!

We reviewed the clone instance this morning and found that multiple OOTB packages (specifically x86 and x64 packages) were in the installed state, so that might be the reason for the issue.

Please ensure that the packages/bundles are correctly installed.
