Adaptive form throws blank screen on edit

AEM: 6.4.8.1

Opening up the adaptive form for editing throws a blank screen. The console log throws 404 on forms.min.js. I see some minifcation errors (screenshot below) in the logs 

We are using yui compressor across envs which is working fine in lowers, its only prod that's causing this issue

Any ideas is much appreciated
