Change a Field Type from Picklist to Open Text in a Dependent Field

We are trying to build a form with the following scenario.  We have 30 countries that have set State/Provinces. We have these queued up and ready to go with the picklist values.  However, if someone selects a country that does not have set State/Provinces, we need the user to be able to type the value, thus switching the field type to open text. Is this possible?

You can set up the form with conditional visibility for a couple of fields. The first would be to make state/province visible when country is Canada or United States (and any additional countries that you have a picklist for). You would also set up a custom text field for self-entered state, and make that field visible if specific other countries were selected. 

This is similar to what you've suggested above, but uses two different field forms if depending if you have a picklist for that country or not.
