Recommendations in an XT - Status Meanings

Morning Community, we are using the XT function to deliver our recommendations as per https://docs.adobe.com/content/help/en/target/using/recommendations/recommendations-as-an-offer.html#status

But this doesn't explain the status and how to correct.

1. When we have Recommendation Ready what does the date next to the status signify and how is this updated?

2. How long should the Fetching Results be visible. We have examples that have been in this status for weeks.

3. Recommendation Not Ready - What does this actually mean? How do we get the recommendation to be ready?
