How do I synch leads to Salesforce upon form submission?

I've setup my campaign so that upon form submission I receive an email alert and then the lead gets synch to SFDC. It was working last week and is now suddenly not functioning. Any insight?

What part of it isn't working? The SFDC sync or the alert? Do you have it set up so that each lead can run through the flow every time?
