Wordpress / Gravity Forms / Marketo Integration

I have successfully connected a Gravity Forms created in Wordpress to Marketo using the Marketo plugin by Zach Katz.
I am able to submit the form and the entry appears in a particular campaign.
The issue is that if there is already an entry in the campaign a second entry does not show up.  I am submitting completely different data each time. If I delete the entry in Marketo a new submission from Gravity Forms shows up in Marketo.

Any thoughts?

What are the fields & hidden fields you are using?

Nothing hidden on this one.

This form submits to a second form which has the same 3 fields which are hidden and shows 2 additional. (Don't ask why they are doing it this way - I know it's bad)

If I was to delete the current lead entry in Marketo and submit this form, the entry would make it's way into Marketo, but if not, the entry will get submitted into Gravity Forms backend, but not into Marketo.
