What are the possible meanings / interpretations for a Campaign with Campaign Type =  "Batch Campaign - Never Ran"?

Could this mean that the batch campaign used to be a Trigger Campaign (which might have generated results) but was since converted to a Batch Campaign that was never run?

In other words, could a "Batch Campaign - Never Ran" ever have Members?

We are cleaning up our Marketo instance and I have a number of questions around Campaign Member Counts vs Program Member Counts, so if there is a good resource available with this information, a link would be greatly appreciated.

Like here?

It may well mean that it was a trigger campaign that has since been converted to a batch campaign.

You can check to see the history of the smart campaign (even in previous iterations of it), by clicking on the results tab:

A never run Batch hasn't run yet. If you do Campaign Inspector filtering, then this will show you all the batches that just never ran. Generally, these should have Members=0

I suppose it's possible if a trigger were converted, it might show this.

Please remember that Smart Campaign Membership is based on the smart list qualification rules at the time it was ON or batched and run. This has NOTHING to do with Program Status unless the Flow had Change Program Status.
