Visitors Complete Marketo Form Yet No Web Activity Logged

We are experiencing the issue where a visitor to our landing pages completes a form but Marketo does not log any web activity.

Over 25% of our form fills do not have any corresponding web activity logged in Marketo.

I find it VERY difficult to believe that this percentage of visitors have Javascript turned off or are blocking cookies.

Can you please provide the URL of the LP? We would be able to troubleshot the JS code and check whether something prevents from logging web page visits.
