Value list do not appear on Google Chrome.

I have created a form where one of the field's Input type is select and value list contains 5 values.

When viewing approved page on Chrome, the user is unable to open the dropdown list and pick a value.

When opening on Firefox it works perfectly.

Note:
1. This problem appears only when this field is on the top of the form and comes before all other fields
2. I used a blank template to create this form.
