unable to find embed code for a form

I am unable to find the "Embed code" or "Approve" options for a form in our Marketo instance.

Background :-

This form was created way back in 2014 and was embedded on our website. Which means the form was approved and the embed code option was available at that point of time. I also tried to submit an inquiry on this form present on our website (today) and I am able to see the form fill out information on the lead (activity log). But this form is currently in unapproved status and doesn't have embed code option.

I would bet your forms is a V1 form.

Forms 2.0 was launched early 2014, so if your instance is older, you may have v1 forms still there.

You will have to recreate it in the v2.
