Track changes in design?

An email was sent, but the Sample that was sent beforehand was different!

We would like to know when, and by who, were the changes to the email effected - between sample and launch.

Is there any log of activities or access requests - on the program or editor?

Are you sure the edits were re-approved?
Did someone use the Headstart method?
Admin > Audit Trail can help, although I've had trouble using it for some situations. It may only show you who Edited the Asset and when.
