Token to capture form name

We have placed marketo forms on our websites, we have different form for different language, Is there a way to create a token based on the form name so that it can be included in the notification email to the sales guys.

I went through this article about Tokens for Interesting Moments http://community.marketo.com/MarketoArticle?id=kA050000000KywJCAS , but couldn’t figure it out.

Trigger tokens described in the article at http://community.marketo.com/MarketoArticle?id=kA050000000KywJCAS are only available for Interesting Moment flow action.  You should consider using the "Original Source Info" field which captures the source through which the lead first became known.  As you have different forms for the languages, you can use the {{lead.Original Source Info}} in your emails to indicate which form was used.

If you set up each language's form in a different program you use the {{lead.Acquisition Program}} token which will tell you which form they filled out.

We handle this issue by creating a form for each language (Contact Us-Spanish, Contact Us-English, Contact Us-French), creating a landing page for each individual country and creating a seperate alert email for each country that says in text which form/page/country the lead is from and providing the rest of the info in tokens like name, company title, etc..

Alert Smart Campaign Example:
If filled out form Spanish Contact Us and web page was Contact Us Mexico send Alert Mexico Contact Us to Sales.

You may consider tagging language on the leads based on their form fillout, then including the value of this field in your interesting moments.  This will enable you to also monitor performance by language over time which is a likely candidate for leads not receiving immediate follow up.
