Token Showing in Email

I sent out an email campaign that included this token {{lead.First Name:default=} and received a response back from a lead who was able to see the token in her email. In Marketo/Salesforce, her name is correctly mapped to the first name field, and I purposefully left the default blank in case there was a missing field. When I tested the email, all looked normal.

How can I prevent this from happening again?

Test your emails before sending them out.

At the least, hit Preview.

You took out one of the closing curly braces which caused the issue.  Also, you can remove the entire :default= part if you're not using a default.
