Not sure I understand your question David.
Are you looking for a list of countries and states to import to Forms?

Edward: I would like to Tag programs based on country or state. I don't want to spend the time setting up ~200 countries and 50 states if this already exists. It seems like this would be used by many Marketo users in the past as well as in the future.

Then you need to set up Country and State tags in your Tag section in the Admin area. Then update all your existing programs with the new values.

Josh's list will help you speed up the set up.
