Some results missing from form

I've tested my form to see if I can receive the results and I have started a smart campaign to receive it by email, however, it seems some results are not being captured. For example, I have a  field for leads to opt in or out of additional information from us and I can't see these results. All I can see is the name and email from that lead. Would be appreciated if anybody could help. Thanks

You will need to include those field tokens in your Alert. Something like:

First Name: {{lead.First Name}}
Last Name: {{lead.Last Name}}
Marketing Preference: {{lead.YOUR FIELD'S NAME HERE}}

The current token I have is SP_Send_Alert. Is that not right?

Hi Poonam, that is one token that you're able to use, but it won't automatically pull in all of the fields that are on your form. It will just pull in specific campaign fields. The tokens that Jason has suggested will also be necessary in the alert email that you are sending to yourself.

If you want to create a list of all of the results, then you can view the members of the campaign, and update the View to show you the fields that you're looking for, as shown below.
