Smart List using SFDC database and group list

I am trying to deploy a campaign with a Smart List to include SFDC contacts as well as a group list I imported. Criteria 1-4 gives me 1,140 leads. When I add the ACCME list (of 64 names) it brings my lead count to 0.  What am I doing wrong with the below smart list criteria? Filters are set at ALL.

You probably have it set to "Use all Filters."

If you want the 64 in addition to the 1,140, set the filters to:

(1 and 2 and 3 and 4) or 5
