Smart Campaign incorrectly pulling in Leads

I have a Smart Campaign setup to match Leads that filled out one specific form, any other form, and where Company is not empty.  There have been 4 people that filled out the specific form but the results show 88 Leads.  I have manually checked and they have NOT filled out this particular form.  I am using the ALL filters.  What could be causing this?

Christine would you mind posting a screenshot of the Smart List filters you're using for this?

Do you have the one specific form in its own separate "Filled Out Form" filter?
