What does No Template mean in the Sales Insight Performance Report / email name column?

Does it represent emails sent from Outlook using the Send and Track feature?

It means when reps select Send Marketo Email (either in Salesforce or via the Outlook plug-in), they don't pick any of the pre-loaded templates. They probably just want to send very targeted track-able emails to a lead. Our reps usually do this to follow up with a prospect after a conversation or confirm details they agreed on, confirm times for a meeting, etc.
