Responsive forms

Hi there, I'm trying to add a responsive form to our blog so it can be used in the right rail (stacked) and on our homepage (elements side by side) but I'm having trouble getting our form to display correctly. Right now it is a field and submit button that stagger instead of being side by side or stacked.  See screenshot
