Referral form fields won't show up in an email alert

Hi, does anyone know why the form fields won't show up in an email alert?
The "Referral name" won't show up in Marketo or the email alert we set up. It just says "N/A".

Is this a custom field that you have on your form? Are you able to place the field onto the email alert and the Value associated with this field is "NA"?  Is it a required field?
