Rating Content Widget?

Hello - I'm wondering if anyone knows how to add a feature/plug-in onto a piece of content which allows the reader to rate the piece of content? Ex. "How useful did you find this content on a scale of 1-5?"  The user would then select a number, and the results would be viewed somewhere within the Marketo system.

You can do this with a form.  Just add the form and name it appropriately and then run a report on it later.
