Pushing one form to multiple places in SFDC

I have 2 sections in my form, referrer and referrer. Right now when syncing to SFDC, everything pushes to a new lead. Is there a way to push the referrer portion to a new contact and the referee portion to a new lead? Or would I have to create 2 separate forms for that?

You can absolutely do this from a single form... have you searched the Community for "referral form"? We've discussed this a lot.
